CMS offers simplified enrollment form

The U.S. Centers for Medicare and Medicaid Services (CMS) has approved a simplified form for dentists and other healthcare providers to order Medicare-covered items or refer Medicare patients for covered services, according to a story in the ADA News.

With the implementation of Section 6405 of the Affordable Care Act, CMS permits certain physicians and nonphysician practitioners to enroll in the Medicare program for the sole purpose of ordering or referring items or services for Medicare beneficiaries. CMS is now offering form CMS 855O for this purpose.

The newly simplified process does not apply to practitioners already enrolled in Medicare or those enrolling to submit claims.
