These sweepstakes are being conducted by DrBicuspid.com, located at 1350 N. Kolb Road #215, Tucson, AZ 85715.

• Eligibility: You must be a current DrBicuspid.com member and have completed all of the required registration information.



• Promotional period: The promotion begins July 9, 2008, and ends at 5:00 PM PST on July 23, 2008.



• Entry limit: One entry per qualified member fully completing the survey. Incomplete or false entries will be void.



• Selection of winners: Winners will be randomly selected from all qualified entrants on July 24, 2008. Odds of winning a prize depend upon the number of qualified entries received.



• Prizes: Three winners will each receive an iPod Shuffle.



• General conditions: This drawing is void where prohibited and is governed by the laws of the United States. All federal, state, and local laws and regulations apply.



• Winner selection & notification: Winners will be chosen and notified by e-mail within three business days after July 24, 2008.



• Winners list: For notification of the winners, send a SASE to DrBicuspid.com, Attn: Member Drawing, 1350 N. Kolb Road #215, Tucson, AZ 85715. Requests must be received by August 31, 2008.
